Home care agencies are New York's biggest Medicaid billers

The largest billing organization in any of the three states is Public Partnerships LLC, the payment processor for New York's consumer-directed home care program, at $7.2 billion over seven years. California's top billers are county governments; Texas's are home health and case management firms.

New York's list is home care nearly top to bottom. Public Partnerships LLC, the fiscal intermediary that processes payments for New York's Consumer Directed Personal Assistance Program, billed $7.2 billion over seven years, the largest single figure in any state. California's list is dominated by county health departments; Texas's by home health and case management companies.

A big number is not an allegation. These organizations aggregate payments for thousands of caregivers, clinicians, or county programs. The list shows where the money flows, which is exactly what makes billing anomalies findable when they appear.

Method: billing NPIs ranked by total 2018 to 2024 paid amounts within each state; organization names, cities, and specialties from the CMS NPPES registry (July 2026). State assignment uses the provider's NPPES practice location.
